This evaluation is not based on personal medical opinion, but on a systematic analysis of the international scientific literature applied to the specific case — the same methodological approach used in scientific studies and international clinical guidelines.
Professor Dr Migliorini is an orthopaedic and trauma surgeon with clinic and academic appointments in Germany and Italy, with a focus on joint reconstruction, sports medicine, regenerative medicine, and complex multifactorial musculoskeletal diseases.
This consultation service provides an independent orthopaedic second opinion for international patients and medical professionals. The consultation may be delivered as either a written report or a live discussion, but both formats follow the same principle used in scientific research: a structured, method-based evaluation of the available data.
The analysis is conducted according to evidence-based principles, integrating the patient’s specific questions within the broader clinical context. The resulting document is not a direct medical act and does not replace an in-person medical examination. Rather, it constitutes a technical and scientific assessment of the provided documentation, aimed at clarifying diagnostic reasoning, therapeutic options and decision-making pathways.
The purpose of this service is to provide an independent, method-driven evaluation grounded in the most recent scientific evidence to support informed clinical decisions.
When this service is useful
A second opinion is appropriate whenever uncertainty exists regarding the diagnosis, the proposed treatment, or the expected evolution of a clinical condition, or when a patient wishes to better understand the available options before making a decision.
It is particularly valuable if:
- Guidance after multiple consultations with differing medical opinions
- Confirmation of a proposed surgical procedure, evaluation of alternatives, or clarification of indications
- Unclear or conflicting diagnosis, treatment strategy, or prognosis between specialists
- Persistence of symptoms or development of new complaints despite ongoing therapy
- Uncertainty regarding the choice of implant, surgical technique, or therapeutic pathway
- Interpretation of complex radiological or instrumental examinations
- Comparison between therapeutic options with assessment of the risk–benefit profile
- Determination of treatment priority or urgency
- Optimisation of therapeutic strategy, supportive therapy, or home management in fragile or complex patients
- Need to better understand the disease and its potential progression over time
- Clarification of doubts before starting or modifying a treatment
- Verification of the appropriateness of an ongoing therapeutic pathway
What the patient will receive
The patient will receive either a written clinical report (min 500 words) or a dedicated online consultation.
The opinion includes a structured interpretation of the provided medical documentation, verification of diagnostic consistency, a clear explanation of the condition and its possible course, and reasoned therapeutic recommendations. When appropriate, alternative conservative and surgical options are compared, additional investigations may be suggested, and the degree of urgency and therapeutic priorities are outlined.
The report is written in understandable language while maintaining appropriate clinical detail.
Modalities
The evaluation is performed exclusively based on the medical documentation and diagnostic imaging uploaded by the patient.
The outcome is delivered either as a written report or discussed during a dedicated online consultation.
All written medical reports and video consultations are performed personally by Prof. Filippo Migliorini.
No part of the clinical evaluation is delegated to assistants or external collaborators.
Each case is reviewed directly, including imaging assessment, clinical reasoning, and final recommendations.
Timing
The written report is generally delivered within two weeks of complete documentation being received.
The online consultation is scheduled according to mutually agreed availability.
